  • W-9S Form

    The Office of Student Financial Services is required by the Internal Revenue Service to collect this form to verify your Social Security Number/Tax Identification Number.  This information is used when preparing your 1098-T tax statement each year.  You can upload this form using the "Upload Documents" link on the left panel of this page.

  • Student Agreement Form

    The Office of Student Financial Services is required by the Federal Department of Education to confirm how Hanover College may or may not apply a possible credit balance on your student account.  In order for us to continue processing your financial aid, we require that you submit the attached Student Agreement form.  Please be sure to accept/decline both statements and sign the form prior to returning to our office. You can upload this form using the "Upload Documents" link on the left panel of this page.

Financial Aid Document Information

When viewing "Financial Aid Document Tracking," you will see a list of specific documents needed by our office.  If you have missing documents and need a copy of any of these documents, you can click on "Forms" on the left side of this page to access and print those documents which are required.  Print only the documents on your Financial Aid Document Tracking list.   If you need more information or have questions, email the Office of Student Financial Services at financialservices@hanover.edu or call at 812-866-7091.
